Coming in at about 2000 square feet it would appear, to the casual observer to be pretty typical of an independent community pharmacy. But, what owner Steve Maki, PharmD is doing with his team is far from typical. In this episode of PharmacyCrossRoads Maki share his philosophy and some practical ideas on how to turn is staff into a well-oiled healthcare and customer service team. Two years ago, Maki moved his pharmacy into a new, more spacious and more effectively laid out facility and brought in some new technology. Supporting the move and adopting the new technology was a BIG ask for his staff. But by listening, taking advice and delegating tasks things worked out well. Amy Baxter, M.D., Founder & CEO of PainCare Labs. Dr. Baxter first tells us about her career as a pediatric emergency medicine physician conducting pain care research and how that lead her to becoming a medical device inventor and entrepreneur. Dr. Baxter describes how she developed the prototype for Buzzy, a device that uses cooling and mechanical stimulation to numb needle pain in children with needle phobia. Dr. Baxter describes how PainCare Labs evolved to develop VibraCool, a pain care device that uses mechanical stimulation for musculoskeletal pain and post-operative pain after orthopaedic procedures.

Dr. Lucas Berenbrok is a pharmacist educator and researcher at the University of Pittsburgh School of Pharmacy in Pittsburgh, Pennsylvania. He co-led the implementation of outpatient pharmacogenomic services at University of Pittsburg Medical Center. This unique service, called Primary Care Precision Medicine, provides pharmacogenetic and genetic testing to patients in local and surrounding areas. With his colleagues at the University of Pittsburgh, Berenbrok co-created Test2LearnTM, a pharmacogenomics certificate program for pharmacists. Dr. Neda Leonard, CEO of Rx Gene IQ and she is a Dual Board-Certified Pharmacist in Pharmacotherapy and Geriatrics. She has experience in various setting such as Program for All-inclusive Care for Elderly (PACE) where she uses her expertise in medication management and PGx. She is a seasoned speaker locally and nationally on various subject matters such as pharmacogenomics and polypharmacy. She has published in Drug Metabolism Reviews and was featured in THE CONSULTANT PHARMCIST Journal. Currently she partners with a group of Geriatricians/Internal Medicine physicians using a collaborative practice agreement for patients living independently, in assisted living facilities or in skilled nursing facilities. She has many tools in her toolbox to use such as pharmacogenomics, deprescribing, geriatric medication optimization, Part D Comprehensive Medication Management (CMR) and Targeted Medication Review (TMR),and working with various private insurances such as United Healthcare and Optum. Dr. Larry Shapiro is a clinical psychologist that started back in 1990 but quote on quote retired to become a financial advisor. He was having a discussion with his brother one day, who was a career officer in the Army, returning from Afghanistan. That make Dr. Shapiro to re-think about his contributions to his community, so he got his license back, trained in military trauma, and started again as a psychologist in 2014. He started at St. Louis Behavioral Medicine Institute specializing in treatment of OCD, panic disorder, social anxiety disorder, simple phobias, depression, and combat trauma. Last year he attended the annual Boston Trauma Conference and heard for the first time about the use of psychedelic medicines for treatment of trauma. So after a 150 hour certification program at Integrative Psychiatry Institute he became certified in psychedelic-assisted psychotherapy which now is part of his current practice. Just this year in January he went into private practice so he can focus more on psychedelics. He is also an adjunct instructor in the department of psychiatry at Washington University in St. Louis and a clinical consultant for psilocybin research at Healthy Minds Lab at Washington University.
Post-traumatic stress disorder (PTSD) which is one of the most common psychiatric disorders which affects about 8 million adults at some point in their lifetime in the United States. But it is not clear as to why only some people who experience a traumatic event develop PTSD. Some people say it’s a social construct but in the largest and most diverse genetic study of PTSD to date, scientists from University of California San Diego School of Medicine and more than 130 additional institutions participating in the Psychiatric Genomics Consortium found that PTSD has a strong genetic component similar to other psychiatric disorders. Genetics seem to accounts for between 5 and 20 percent of the variability in PTSD risk following a traumatic event.

Today we are hoping to make sense of the Inflation Reduction Act which is designed to protect Medicare recipients from catastrophic drug costs by phasing in a cap for out-of-pocket costs and establishing a$35 cap for a month's supply of insulin. And, as an historic win, Medicare will be able to negotiate prices for high-cost drugs for the first time ever.

Mark is the Executive Director of America's Agenda, an organization he’s led since its founding in 2005. America's Agenda is a national healthcare alliance that brings together labor unions, businesses, healthcare providers, and policymakers who share a common commitment to smart policy and effective action to advance our mission of affordable, high-quality, readily accessible healthcare for every American. Mark currently serves on the State Health Benefits Value and is a former member of the Quality Task Force in New Jersey and previously worked as an economics professor as well as national director of organizing for the American Association of University Professors. He completed his graduate studies at the London School of Economics.
A major focus for Mark in recent years has been the Pharmacy Benefit Manager issues facing our country. Mark is a founding member of the PBM Accountability Project. He works tirelessly to educate the public and advance solutions to help redirect prescription drug savings from very high PBM profits back to patients, employee health plans, and taxpayers.
Antonio Ciaccia
Antonio Ciaccia (cha-cha) is the President of 3 Axis Advisors, a firm that among many things studies drug pricing. He was recently featured on NBC’s Today Show talking about the lengths PBMs force independent pharmacies to go to find savings for their patients. He’s also spoken publicly about PBMs in the context of the Inflation Reduction Act. Antonio has been in and around pharmacies his entire life. A graduate from The Ohio State University in 2007 with dual degrees in communications and political science. For six years he headed up government affairs for the Ohio Pharmacists Association. After years of studying the pharmacy marketplace, Antonio became increasingly perplexed and concerned as he saw drug costs spiking while payouts to pharmacies were declining. Knowing something was being lost somewhere in the middle of an ever-growing transaction, Antonio has spent years working to crack the drug pricing code and pull the rug out from what he believes is one of the most dysfunctional marketplaces in the world.

Dr. Nardine Nakhla's career has had one common denominator since her graduation in 2007 from Albany College of Pharmacy and Health Sciences — striving to help as many people as possible. That desire is seen in her work with students, as a faculty member at the University of Waterloo's School of Pharmacy. Dr. Nakhla’s patient-centered approach also comes through in her research, which has focused on issues like smoking cessation and high-risk, non-prescription drug interactions in older people. Her professional writing also reflects her desire to help patients. Dr. Nakhla has authored four chapters for the Canadian Minor Ailments reference, one chapter for the American Handbook of Nonprescription Drugs, and one chapter for the Encyclopedia of Pharmacy Practice and Clinical Pharmacy.
In this episode of Occupation Station, Dr. Nakhla talks about how her career path evolved. She entered ACPHS thinking that she would devote herself to community practice. While she and her husband do own a community pharmacy, she has achieved so much more than she initially envisioned. Hear why she calls her time at ACPHS “magical” and how the school has led to lifelong friendships and meaningful connections with professors.
