Output Medical was founded by Dr. Jay K Joshi as an Intern managing complex Intensive Care Unit (ICU) patients. Concerned by the limitations of manual urine output measurements and subsequent kidney care management, he set out to develop a technology that would improve the measuring and recording of urine output to help patients with kidney injury or kidney failure.
Born out of a need to improve urine output measurements, Output Medical is passionate about kidney management and saving patient lives.
Abnormally low urine output (known as oliguria) is a leading indicator of acute kidney injury (AKI) among ICU patients. AKI is a rapid and dangerous loss of kidney function which seriously impacts a patient's quality of care: it increases mortality, cost of care, length of stay, and readmission. AKI occurs in 20% of all ICU admissions each year, meaning approximately two million patients in the U.S. for whom regular and automatic urine output monitoring could improve their health. The average additional cost of a patient with AKI is nearly $16,000, a significant cost to patients and hospitals.

Pharmacogenomics and Provider Status with Scott Knoer | PGX for Pharmacists

According to the National Community Pharmacists Association (NCPA), community pharmacists are highly accessible health care professionals and 95% of Americans live within just 5 miles of a pharmacy. (https://ncpa.org/provider-status) And what APhA or American Pharmacist Association, the largest association of pharmacists in the US is doing is laser focusing on getting provider status for pharmacists. So why is that important when it comes to PGx as this is a PGx podcast? Pharmacists can order the PGx test when they see fit for their patients without the need of a clinician to sign that form. 
Scott Knoer, MS, PharmD, FASHP, became the 13th Executive Vice President and Chief Executive Officer of the American Pharmacists Association in June 2020, after serving for nine years as the Chief Pharmacy Officer at the Cleveland Clinic.
Dr. Knoer is a passionate advocate for patients and the profession of pharmacy. He has successfully lobbied boards of pharmacy in Ohio and Minnesota to allow the advancement of pharmacy practice, and he has led initiatives that changed Ohio and federal law related to the elevation of pharmacy practice and reducing the impact of drug shortages. He received his BA in psychology from Creighton University and his PharmD from the University of Nebraska.

Right to Therapeutic Choice | DarshanTalks

In this episode of DarshanTalks Podcast, the host Darshan Kulkarni Pharm.D, MS, Esq. and Guest Speaker Lewis A. Grossman discuss the Right to Therapeutic Choice. Lewis A. Grossman's recently launched book "Choose Your Medicine: Freedom of Therapeutic Choice in America" on the freedom of medicine in the USA. Lewis Grossman, Author, Counsel at Covington & Burling LLP (https://www.cov.com/) and Professor of Law at American University, Washington College of Law, had spoken about government interference in the 19th century on the healthcare industry.
The conversation started off with the expert explaining his newly launched book based on freedom of therapeutic choices in America that is affected in American law and policy. The compelled speech doctrine sets out the principle that the government cannot force an individual or group to support certain expressions, this was discussed with compelled action which initially the host gave an example of how cigarette advertisements portray and mention that cigarettes are injurious to health whilst asking the expert to share his insights over this topic. He explains his constitutional law perspective on this subject that what government can make you do and what it cannot, he also mentioned about commercial speech is not protected as supposed to political speech or artistic speech.
The conversation also included an interesting topic of off-labeling drugs and ancient drugs like hydroxychloroquine and ivermectin used to treat Covid-19. Furthermore, if the branded company would have endorsed or used these drugs the implications are discussed in this podcast. Also, the subject of monoclonal antibodies was discussed, monoclonal antibodies are proteins created in a lab. The proteins mimic your immune system and are meant to make COVID-19 disease milder in patients who are already sick. As per the expert, the monoclonal antibodies are disseminated by EUA and not FDA approved, highly expensive. They talked about the history of sciences, pharma history, and much more differentiating with the bio-hacking, monoclonal antibodies, hydroxychloroquine, and ivermectin being in the same spectrum. In the mid-segment, what constitutes to be bad-advertisements and how the FDA now is disseminating about bad-ad to the medical industry.

The FDA on Compounding Pharmacy | Pharmacy Podcast Nation

In the past couple of decades compounding pharmacies have been under attack by the FDA. The FDA and compounding pharmacies are supposed to be separated unless the medications that compounding pharmacies are using are found to put the public at risk. Drug companies can only produce medications if allowed by the FDA after they have supplied years of work and evidence proving that the drug is effective and safe. In this way the FDA’s job is clear and very important. The beauty of compounding is that it’s not limited to studies here in the United States. Many compounds are based off of old medications and combinations that have simply been “grandfathered” and do not have any profit benefit for large pharmaceutical companies. Other compounds are based off of other studies that have not been submitted to the FDA and other studies are from other parts of the world, like Europe. By no means are the treatments we use in compounding pharmacies snake oil or hocus pocus.
The compounding of medications is an age-old art dating back as far as the ancient Egyptians. A compounding pharmacist uses raw chemicals, powders and special equipment to make medications. This results in a customized medication made by a pharmacist according to a doctor’s directions to meet an individual patient need.

Pharmacogenomics Market Access and Reimbursement Series

Most of us understand that the reimbursement for pharmacogenomics tests has been one barrier amongst many barriers to pharmacogenomics’ wide-scale clinical adoption. Finding myself highly frustrated with the slow clinical uptake by PGx test ordering by healthcare providers back in 2017, I brainstormed about which payers were the most vested in improving patients’ medication outcomes and who would therefore more likely to listen to my pitch about PGX decreasing adverse drug events and increasing therapeutic medication outcomes.
As a consultant to self-insured employers who were seeking to lower their employees’ drug spend, I found a warm and receptive audience for pharmacogenomics. It was through my relationships with self-insured employers that I met today’s guest.
Again, fate intervened, and today’s guest reached out to me via LinkedIn. She had been following my pharmacogenomics posts and liking what she had seen she introduced me to a group of pharmacists in Kentucky who were integrating PGx into their PBM via comprehensive medication management.

Jane Cheshire Gilbert, CPA Director of Retiree Health Care for the Teachers’ Retirement System of the State of Kentucky Jane has served TRS retirees since April 2002. She manages two retiree health plans covering 48,000 retirees. She also serves as a leader in the areas of health insurance cost containment, project management, risk management and federal health care solutions.
Gilbert served in management and directorship positions for a Louisville, Kentucky law firm and a cost containment company, The Rawlings Company, from 1989 through 2002. Prior to that, she worked as an accountant for a national CPA firm. Gilbert earned a bachelor’s degree in accounting from Bellarmine University in Louisville, Kentucky and is a certified public accountant and a certified government benefits administrator. She has served on the board of the State and Local Government Benefits Association and is a member of the Public Sector Healthcare Roundtable.
