Medical Affairs will be discussed in this episode of the DarshanTalks podcast. Host Darshan Kulkarni welcomes guest Irene von Hennigs. Irene von Hennigs, Pharma D who is Vice President Medical Affairs at ChemoCentryx talks about her journey and specifically about medical affairs. She is been in the pharma industry and medical affairs for a period of 26 years. While in her consulting career she started her own company partnering with pharmaceutical companies and ChemoCentryx was one of them. There are pros and cons to consulting versus being on board, consulting business was at first for her, and held that position for about six months in ChemoCentryx. She was keen on building medical affairs and fascinated by the science of it. A pertinent question was asked - how do you build a medical affairs team? - on this, the expert answered it depends on the structures, procedures, priorities, and recruitment ( building the team and bringing in the right individuals) She mentioned the process of the structure that has been undertaken by the organization. The one-word answer was 'data', she also mentioned usage of clinical trials data and extracting it, but the prominent part is the right minds putting the effort while finding and training such minds enables building a strong team.
The complement science, also known as complement cascade, is a part of the immune system that enhances (complements) the ability of antibodies and phagocytic cells to clear microbes and damaged cells from an organism, promote inflammation, or immune oneself from the infection. They then talked about building relationships and healthcare is a relationship business for eg., physician relationship with the patient, company's relationship with the physician, etc. Speaking of relationship-building she explained about the company focusing on various databases like publications, speaking engagement, medical leadership, academic institutions, centers of excellence for disease state and one can compile a list for this purpose. Collecting and forming a certain list is performed by any company however giving a human touch and tweaking the list is of utmost importance for building relationships thereon. In the mid-segment, it was asked how GDPR impacts the formation of the company, and the regulatory side was discussed. In closing, she talked about the field medical team should include the right team, the right talent, they have to have the right training and the resources to represent the company

The role of the community pharmacist is changing as millions of Americans rely on community pharmacists to provide a new level of care during this healthcare challenged time. Independent pharmacists are able to take active roles to improve outcomes for their patients and lower costs for payers and patients alike.
Todays special guest is Dr. Rina Shah, PharmD. Rina is Group Vice President of Pharmacy Operations and Services at Walgreens. Her passion for pharmacy has been centered around transforming patient care to make an impact on patients’ lives as well as empowering those around her to do more.
In her current role, Rina leads a team that is responsible for pharmacy operations, services and specialty pharmacy across 9,000+ Walgreens pharmacies. In the last year, Rina has led Walgreens pharmacy response to COVID-19, implementing changes such as waiving prescription delivery fees, initiating proactive outreach by pharmacists to vulnerable patients, and enhancing team member and customer safety protocols. In addition to her current role, she now leads pharmacy operations for COVID-19 vaccination services.
Rina helped launch Feel More Like You in 2019, a health and beauty service available at no cost to help people living with cancer manage the medical and physical changes associated with cancer treatment. She also spearheaded the creation of a new team of dedicated pharmacists at Walgreens called Health Outcomes Pharmacists. Often outside of a traditional retail setting, these specialized pharmacists work closely with patients most at-risk of medication non-adherence to address barriers that could prevent them from taking their medications as prescribed. In 2020, Rina led the pharmacy team that secured the first enterprise-wide URAC accreditation in Specialty Pharmacy for more than 300 community-based specialty pharmacies. Often patients with life-changing health challenges need support from their pharmacy team in their local community, this accreditation assures patients of the high standards of care in place as they access their medications and understand their treatment.
She began her career at Walgreens in 1998, where she held various pharmacy roles in store. In 2007, Rina joined the corporate support center to lead several pharmacy-specific initiatives across the organization, including the launch of the Walgreens immunizations program chain-wide. The program increased immunization rates among customers covering 7 million people. She has also led sales operations and client services within market access.
Rina was recognized by Crain’s Chicago Business as one of “40 under 40” for her leadership at Walgreens, by Fierce Healthcare as one of 2020’s Most Influential Minority Executives, and by Drug Store News as one of the Top Women in Retail for her business excellence. Rina is actively involved with the UIC College of Pharmacy, APhA, NASP, the CDC and NACDS.
She received her Doctorate in Pharmacy from the University of Illinois at Chicago in 2005.

Dr. Scott Knoer, PharmD is the CEO of the American Pharmacists Association. Scott has been very vocal about the FDA eliminating barriers to Pharmacists prescribing COVID antivirals and how the American Medical Association (AMA) seeming to be more concerned with their 'turf' and political position compared to the benefit to public health within America.
Amesh Adalja, MD wrote in MedPage Today, an article titled: "A Successful 'Test-to-Treat' Program Requires All Hands on Deck"
Within the article, Dr. Adalja wrote. In encouraging news, President Biden just announced a new plan to increase faster access: test-to-treat. The plan will enable pharmacies to become one-stop shops for COVID-19 diagnosis and oral antiviral treatment. Having treatment available at the point-of-diagnosis has the potential to significantly improve prescribing rates, which will have the downstream impact of reducing hospitalizations. More sick patients will quickly be able to access treatment.
"We can't have a guild-like mentality that seeks to use government force to exclude qualified persons from engaging in a field for which the AMA wants physicians to be the exclusive purveyors. This has been evident in organized medicine's general opposition to expanded practice scopes for nurse practitioners, physician assistants, psychologists, and -- as is clear -- pharmacists. It's bad enough to advocate for this on a day-to-day basis. To do so during a public health emergency, where all hands are needed on deck, is destructive." -- Amesh Adalja, MD

Pennsylvanians suffer while pharmacy middlemen profit

Pennsylvanians are getting crushed by the costs they face at their neighborhood pharmacy counter. The situation has grown so bad that a shocking one in five Keystone State residents report they are unable to take their medications as prescribed due to the cost burden.[1]
As a lawmaker and an advocate who speaks to patients in Pennsylvania every day with liver disease, diabetes and other chronic conditions, I am convinced more strongly than ever that we need to reform the prescription drug supply chain.
To understand how we got here we need to understand what’s driving increasing costs. A new report found that, in 2020, manufacturers of brand-name and generic medications received only 37 cents of each dollar of the more than $680 billion spent on medications. If you look exclusively at manufacturers of brand-name medications, the drug makers are taking in just under 50 cents on the dollar. So where is the money going? Turns out that it’s little-known middlemen corporations, Pharmacy Benefit Managers (PBMs), that are taking a bigger and bigger share of the pie.[2]
PBMs were created to negotiate with insurers and drug manufacturers and establish which prescription drugs are covered by a health plan. In reality, they demand steep discounts from drug makers off of the original list price as a condition to include those drugs on the approved list of medications – and then keep the savings for themselves. But here's the real rip-off: Patients at the pharmacy counter still face co-insurance charges based on the list price -- not the discounted price insurers actually pay. In other words, PBMs are enriching themselves while hiking prices for the rest of us. {3}
Worse still, PBMs manipulate their powerful market position to reduce the availability of prescription medications by excluding them from formularies. PBMs make their money when they demand exorbitant rebates from drugmakers and charge patients full cost, meaning they prefer to include drugs with higher list prices on their formulary over an equally effective drug with a lower list price. As a result, a number of drugs are excluded from formularies because PBMs can’t make as much money from them. In 2020, the three largest PBMs excluded 846 FDA-approved drugs, denying patients access to more medications than ever before.
This market manipulation has disastrous results for patients. A study published last year found that these practices, called “drug utilization management,” can lead patients to abandon their prescribed medications and delay treatment. Approximately 20% of prescriptions in the U.S. are never filled, according to the study.[4] This outcome is simply unacceptable, and our leaders need to take action.
Congress needs to ensure that any attempt to rein in out-of-pocket prescription drug costs includes taking a close and hard look at PBMs. Doing so would go a long way toward ensuring every patient and healthcare consumer in our state can access and afford to take their medications as prescribed by their doctor.

He has over 28 years of leadership, business development, marketing, product management, assessing long and short-term needs, establishing objectives, improving organizational performance, and resource management. Expertise in medical device regulations, product development processes, software engineering, embedded systems, wireless communication systems, and robotics and control systems. Masters in Electronics Engineering, MBA in International Business. Six Sigma, Black-belt certified. With all the bucket of knowledge of digital health, the conversation started off with FDA going through massive changes currently. He explains his area of work in the digital health technology sphere and leading digital health with no jurisdiction boundaries, as this sector is wide in nature, and he further explains how it's always been fun to work in such an atmosphere. When it comes down to FDA there are portions of that spectrum of digital health as it may be general wellness. According to the expert, digital health is a spectrum, a continuum, a journey for an individual and purview under FDA's jurisdiction. The focus of the conversation was on the department that the expert works and the people who don't know much about can reach out to the department for advice, suggestions, reviews or portions of review depending on the products that come to different parts of the agency. There are some policy challenges, he emphasised on FDA needs to start evolving that their agency have been actively working on with other parts of the agency. Recent guidance on digital health tech in clinical trials was launched by his department and he described all the related information and implications in this podcast.
