Being rich in American is a stoic title. Being among the wealthiest within one of the wealthiest countries in the world is a common financial goal. But what is rich? Who gets to define it? And should we all being pursing that same definition?
Most Americans say that to be considered wealthy in America, you need 1.9 million in Net Worth. But that range changes depending on who you are talking too and what generation they fall into. The specific way of defining being rich also changes a lot depending on the person you are speaking with and what generation they fall into.
In this episode, Derek breaks down his two definitions of rich, how much you need to have to be considered rich by your peers, and why we should all be changing the way we evaluate wealth in America.

According to the American Association for Cancer Research, 80-90% of the administered dose of the cancer medication 5-fluorouracil of 5FU is degraded by the enzyme dihydropyridine dehydrogenase or DPD for short.

According to the Institute of Safe Medication Practices (www.ismp.org/resources/screening-dihydropyrimidine-dehydrogenase-dpd-deficiency-fluorouracil-patients-why-not) the incidence of DPD deficiency is low like 1-7% of the population but the consequences can be unfortunately fatal for those few.
Christina Delsoldato, wife and mom of 3 beautiful children has spent most of her life laughing and loving as much as she can. Christina has built a successful professional career in hospitality, customer service and sales. Christina has faced many obstacles and traumatic events during her life. Christina took her mother’s quickly declining health into her own hands. Despite having no clinical and medical background, she spent countless nights reading, learning and researching to find answers.
These efforts and findings are what allowed her beloved mother to come home to spend her last days on earth surrounded with the love and support of her closest family. Since her mother’s diagnosis with Pancreatic Cancer and 5FU toxicity, Christina has become determined to help guide others to self-educate, research and ask questions about potential life-threatening side effects that could indeed be prevented and/or reversed.
Tragically, only three months after her mom passed away, her father died suddenly. Christina is on a mission to empathize and advocate not just for her family, but for anyone in need.

Spruce Mountain Pharmacy is located in the beautiful rural country side in the town of, Jay, Maine. Coming in at about 2000 square feet it would appear, to the casual observer to be pretty typical of an independent community pharmacy. But, what owner Steve Maki, PharmD is doing with his team is far from typical. In this episode of PharmacyCrossRoads Maki share his philosophy and some practical ideas on how to turn is staff into a well-oiled healthcare and customer service team. Two years ago, Maki moved his pharmacy into a new, more spacious and more effectively laid out facility and brought in some new technology. Supporting the move and adopting the new technology was a BIG ask for his staff. But by listening, taking advice and delegating tasks things worked out well.

Diet is a significant portion of diabetes management that is unfortunately sometimes overlooked. An article recently published in 2018, Diabetes and Diet: A Patient and Dietitian's Perspective, discusses the challenges that Type 2 Diabetes patients face when trying to lose weight, especially while taking insulin therapy. The dietitian plays a vital role in the management of carbohydrate awareness while concurrently minimizing the risk of hypoglycemia. Because of this, utilizing the dietitian in DSMES classes can have a positive impact on patients' health. Isabelle Litvak, PharmD & Rachel Hay, PharmD are the new hosts of season 2 of Beyond the Sig for the DSMES Podcast Series powered by the Pennsylvania Pharmacists Association. Dramatic Increases in CLIA Certificates of Waiver Demonstrate New Trend in Pharmacy Service. New research shows that consumer demand for COVID-19 testing has driven pharmacies to become nation’s second-largest provider of CLIA-waived tests
CLIA Waivers Background:
The U.S. Clinical Laboratory Improvement Amendments of 1988 (CLIA) enabled greater access to simple, low-risk (CLIA-waived) tests by allowing their use in facilities such as pharmacies, long-term-care facilities, and physician offices. To administer CLIA-waived tests, these facilities must obtain a CLIA certificate of waiver from the U.S. Centers for Medicare and Medicaid Services.
Guests:
Don Klepser, PhD, MBA, and Mike Klepser, PharmD, FCCP, recently published in Research in Social and Administrative Pharmacy, showing that pharmacies are now the nation's second-largest provider of CLIA-waived tests by the total number of locations, trailing only physician offices.
Jason Ausili, FDS Amplicare’s Chief Clinical Officer, is available for media interviews about how community pharmacies can build a Point-of-Care Testing practice during this unprecedented time. Administrative barriers have been relaxed and both the CLIA waiver application and the Provider Transaction Access Number (PTAN) processes have been expedited to help clear the path for those interested.
Challenges:
The number of pharmacies with CLIA certificates of waiver jumped 45% to 15,671 from 2015 to 2020.
Most of the growth occurred between 2019 and 2020, suggesting that surging consumer demand for COVID-19 diagnostic testing was behind the surge in CLIA certificates of waiver.
Solutions:
Pharmacies must embrace the opportunity to further entrench themselves as hubs of healthcare in their communities.
Pharmacies must adopt the operational tools, training and software solutions needed to seamlessly bill payers for CLIA-waived tests, in addition to the patient communication platforms to support engagement.
The COVID-19 pandemic has recently focused a spotlight on CLIA-waived diagnostic testing, as the lack of a vaccine or effective treatment in the early days of the pandemic left diagnostic testing as one of the only tools to control the virus’ spread. To meet this increased patient demand for diagnostic testing, the federal government authorized licensed pharmacists to order and administer authorized COVID-19 tests, attributing the decision to the close relationship pharmacists have with patients and healthcare providers as well as the convenient proximity of pharmacies to most Americans.

Jamie grew up in a small town in Wyoming where she got her start in pharmacy working at the local City Drug. She attended the University of Wyoming School of Pharmacy for her Doctor of Pharmacy. For 10 years post-graduation Jamie worked in community pharmacy while having and raising her 4 little boys. However, a young family and community pharmacy work schedules are not highly compatible, so in 2020 Jamie created her own job for herself that better suits her talents, her passions, and her family’s busy schedule. She abruptly jumped out of retail and into the world of entrepreneurship and has found her true calling in life. She is the founder & CEO of Arches Health, LLC, and serves there as a precision medicine consultant pharmacist focusing on pharmacogenomics.
After getting her practice up and running she turned around and began promoting and empowering pharmacists to champion PGx and create their own consulting practices through her PGx Consulting Confidence Academy. Dr. Wilkey is a member of CPIC as well as the nonprofit organization GTMR.
